Does the concept proposed here really offer advantages compared with the existing means of communication, e.g. journals, congresses, and will it lead to the main objective of improved patient care in Nepal.

I wonder. The Nepalese colleagues must decide in the end. However, if one has internet access there is quite an amount of information available for free if one for instance checks out all the organisations mentioned above (technological possibilities).
There are also means to get low cost or free access to international medical information (see Ptolomeaus - project of Canadian Network International Societies of Surgeons Or ‘PLoS Medicine (http://www.plosmedicine.org), the new open-access international medical journal from the non-profit Public Library of Science (http://www.plos.org)’.
A real advantage I could see in networking surgeons in Nepal and other countries. So it would be much more than a website at the end.
